Renewal Membership Application

If you would like to apply for your membership online, go to our Membership Application form. And fill out all appropriate information that is needed for our FFAA Membership Trustee to submit to our Association. Once you have filled out all the necessary parts, you will be taken to our payment page selection you may choose "Payment by Paypal" to pay for your membership fee ($25.00) payable with your credit card, check card, or e-check via Paypal.

Or you can choose to select our "Mail-In" format, where you can print out the form and mail it to our FFAA Membership Trustee.

"PDF Mail-In" your membership application in PDF format. Please print out this PDF Application Form and submit your form & along with your check or money order.

If you are a returning member and wish to submit your renewal membership online. You can now choose our Online Renewal Form and pay by Paypal or you can mail-in an online printed format or there is a PDF format of the renewal form. Please choose from the following options below:
